Lincolnshire PCT, Lincolnshire Sports Partnership and SKDC partnership scheme.
In support of the Active4Life brand we have developed, with support of the Lincolnshire Primary Care Trust and the Lincolnshire Sports Partnership, the South Kesteven Exercise Referral Scheme. The South Kesteven Exercise Referral Scheme introduces people to the benefits of physical activity. Everyone can benefit from being more active but this scheme is for South Kesteven residents, who may already have a pre-existing medical condition, which could benefit from exercise or those at risk through a non-active lifestyle.
The Exercise Referral Scheme provides two different programs, administered by appropriately registered exercise professionals, to meet the clinical needs of patients dependent on their health and physical condition.
The scheme is for adults aged 16 years and over.
How the Scheme Operates:
The scheme operates with local GP surgeries who have signed as partners. If a GP feels a patients would benefit from physical activity he/she will make a referral to the local Leisure Connection site.
Common reasons for client referral are:
- High Blood Pressure
- Diabetes
- Obesity
- Asthma
- Circulatory Disease
- Bone and joint problems
Once a referral has been made the Physical Activity Advisor at the local site will contact the patient direct for an initial 'one2one'
Questions and Answers
Where are the exercise sessions?
The supervised exercise sessions are held at district leisure centres: Grantham Meres, Bourne, Deepings and Stamford and The Olde Barn Hotel, Marston. Exercise sessions are scheduled at a variety of times and contain a range of physical activity.
How long does it last?
Clients have their own, specifically designed 12 week exercise programme and are required to attend all sessions.
What is the cost?
There is no cost to the Exercise Referral Scheme. Patients will receive a structured 12 week programme and support from the Physical Activity Advisor free of charge.
What will I learn during the programme?
During the sessions you will learn about the positive effects that physical activity can have on your health. You will meet new people and enjoy yourself!
Are these sessions supervised?
Yes, sessions are supervised by a qualified fitness professional, who have a wealth of expertise and experience.
Will there be people like me there?
Yes, Exercise Referral caters for all sections of South Kesteven's diverse population.
What happens when I finish?
By the end of your programme you will have enough fitness skills to be able to exercise independently. To ensure continued benefits from physical activity, you must participate regularly (ideally five times per week for at least 30 minutes per session).
When you complete the programme, the Physical Activity Advisor will recommend follow on activities to maintain and develop your physical activity levels.
How do I get started?
Your GP or practice nurse must recommend you. They can refer you to the Exercise Referral Programme if they are registered. If they are not, contact SKDC/ Leisure & Amenities and a team member will approach your GP to register on your behalf.
